Connect with your primary care provider.

Choose a network primary care provider (PCP) from our provider directory. Your PCP is the leader of your health care team. You and your PCP will work together. The relationship you build is important and will help you along your health and wellness journey.

Visit your PCP for routine care, yearly checkups and other general health concerns. Each member covered under your plan can select his/her own PCP, or you may all choose the same one. You may also choose a pediatrician for your child. Females over the age of 14 may select an OB/GYN in addition to a PCP.

Your PCP will keep a record of your health history to make informed decisions about your care. And, if necessary, he/she will communicate with other doctors, like specialists, to discuss additional testing or treatment. With an HMO plan, your PCP helps coordinate specialty care through a referral.

A primary care provider (PCP) is required with an HMO plan. If you do not choose a PCP, one will be assigned to you based on your address. You can change your PCP at any time.

Make an appointment with your PCP before you experience symptoms. Your plan covers preventive care screenings when you see an in-network provider, even if you have not met your deductible (if one applies to your plan).
